COVID-19 Information for patients

Noosa General Practice is an approved COVID Vaccine Clinic commencing April 2021

COVID Vaccination Clinics will commence 10th April and ongoing every Saturday from 8:30am to 12 noon.

***New patients & patients who haven’t been to the practice in past two years are required to phone the practice for appointment.

Forms that are required to be read or filled in before you go to your appointment for your COVID Vaccination:

Eligibility Declaration Form(You must complete and bring with you if you are a New patient and / or regular patient  <70yrs  in an eligible occupation group)

Patient Consent Form for COVID Vaccine (Please ensure you have read the consent form, you do not need to print and sign. Verbal consent is acceptable on the day)
